Devoted Stewards LLC works collaboratively with families, case managers, discharge planners, and agencies to ensure safe and appropriate residential placement.

Our intake process is structured, transparent, and designed to support smooth transitions.


Who Can Submit a Referral?

• Family Members
• Social Workers
• Case Managers
• Hospital Discharge Planners
• Veteran Service Organizations
• Community Agencies


Basic Eligibility Guidelines

To ensure appropriate placement, individuals must:

• Not require licensed medical or skilled nursing care
• Be medically stable
• Be able to participate in a residential living environment
• Agree to house expectations and guidelines

Each referral is reviewed individually.


Our Placement Process

We prioritize timely communication to reduce placement delays.


Required Documentation (When Applicable)

• Identification
• Funding verification (SSI, SSDI, VA, etc.)
• Discharge paperwork (if applicable)
• Medication list
• Emergency contact information
